What is hyperbolic tapering

If you’ve ever tried to reduce an antidepressant, benzodiazepine, or another psychiatric medication and found that the last few milligrams felt harder than the first few, there may be a reason.

Medication effects are not always proportional to the number of milligrams you take. With some medications, a seemingly small reduction at a low dose can produce a much larger change in the nervous system than the same milligram reduction at a higher dose.

Hyperbolic tapering is an approach designed around this principle. Instead of reducing medication by the same number of milligrams each time, reductions generally become smaller as the dose becomes lower. The goal is to give the brain and body more time to adapt while minimizing withdrawal symptoms.

Why Can Stopping Psychiatric Medication Be Difficult?

When a medication is taken regularly, the nervous system adapts to its presence. Reducing or stopping it can require another period of adjustment.

Depending on the medication and individual, withdrawal symptoms may include:

  • Dizziness or “brain zaps”

  • Anxiety or agitation

  • Insomnia

  • Nausea or gastrointestinal symptoms

  • Mood changes or tearfulness

  • Flu-like sensations

  • Tremor or increased sensitivity to stimulation

  • Rebound symptoms that resemble the original condition

Withdrawal can sometimes be mistaken for a return of depression or anxiety. This distinction matters because the appropriate response may be very different.

How Is Hyperbolic Tapering Different?

Imagine someone takes 20 mg of a medication.

A traditional linear taper might look something like:

20 → 15 → 10 → 5 → 0 mg

Every reduction is 5 mg. But each 5 mg reduction represents an increasingly large percentage of the remaining dose.

Hyperbolic tapering instead uses progressively smaller reductions. A simplified example might look more like:

20 → 18 → 16.2 → 14.6 → 13.1 → 11.8 → ...

The exact schedule is individualized. This example is only meant to demonstrate the concept—not provide a tapering plan.

For some medications, receptor or transporter occupancy follows a hyperbolic rather than linear relationship with dose. This means that relatively small doses may continue to have substantial biological effects. Researchers have therefore proposed progressively smaller reductions as doses become lower so that changes in biological effect are more gradual.

In simple terms: the lower you go, the smaller the steps may need to become.

Hyperbolic Tapering for SSRIs and Other Antidepressants

Hyperbolic tapering has been particularly discussed in relation to SSRIs and other antidepressants.

Research examining serotonin-transporter occupancy suggests that lowering an SSRI from an already-low dose to zero can create a disproportionately large change in pharmacologic effect. Horowitz and Taylor proposed that antidepressants therefore be tapered gradually, hyperbolically, and—when clinically appropriate—to doses below conventional minimum therapeutic doses before discontinuation.

For people who are sensitive to medication changes, this process may take months rather than weeks, and sometimes longer. Observational research on personalized hyperbolic tapering has also found that smaller daily reductions were associated with less withdrawal, although the optimal tapering strategy has not yet been established through large randomized trials.

This approach may be especially worth discussing if you:

  • Have taken an antidepressant for a long time

  • Have previously experienced significant withdrawal

  • Become symptomatic after relatively small dose changes

  • Have difficulty with the final stages of discontinuation

Liquid formulations, carefully divided tablets, or compounded formulations may sometimes allow smaller reductions when commercially available strengths are too large.

Hyperbolic Tapering for Benzodiazepines

Medications such as clonazepam (Klonopin), alprazolam (Xanax), lorazepam (Ativan), and diazepam (Valium) can produce physical dependence when taken regularly. Abrupt discontinuation can cause significant withdrawal and, in some circumstances, can be medically dangerous.

The 2025 Joint Clinical Practice Guideline on Benzodiazepine Tapering recommends individualized, gradual dose reduction rather than abrupt discontinuation for patients likely to be physically dependent. It specifically recognizes hyperbolic tapering as an option for patients experiencing withdrawal symptoms.

For example, rather than repeatedly removing the same number of milligrams, a clinician might calculate each reduction from the current dose. Consequently, the absolute amount removed becomes progressively smaller.

Some patients require particularly slow reductions near the end of treatment. The guideline emphasizes adjusting or pausing the taper when significant withdrawal symptoms emerge rather than forcing everyone through a predetermined schedule.

Benzodiazepine tapering should be medically supervised because severe withdrawal can include seizures and other serious complications.

What About Gabapentin and Pregabalin?

Gabapentinoids can also produce withdrawal symptoms in some people, particularly following regular or prolonged use.

Current prescribing information for gabapentin recommends gradual discontinuation rather than abrupt cessation and specifies tapering over at least one week, with longer periods at the prescriber's discretion.

For patients who experience difficulty with conventional reductions, a slower individualized approach may sometimes be considered. Recent British Association for Psychopharmacology guidance discusses hyperbolic tapering as a potentially useful approach for gabapentinoids, while also acknowledging that available commercial formulations can make very small reductions difficult.

Importantly, the evidence supporting hyperbolic tapering is much stronger conceptually and more developed for antidepressants and benzodiazepines than it is for gabapentin. There is not one scientifically established hyperbolic schedule that should be applied to every gabapentin patient.

What Is Microtapering?

You may also hear the term microtapering.

The concepts overlap but aren't identical.

Hyperbolic tapering describes the shape of the taper—reductions become progressively smaller as the dose decreases.

Microtapering describes the size and frequency of reductions—very small reductions may be made frequently rather than making a larger reduction every several weeks.

The 2025 benzodiazepine guideline notes that some highly sensitive patients report benefiting from microtapering, although it also emphasizes that direct research evaluating this strategy remains limited.

There Is No Universal “10% Rule”

You may encounter advice online recommending that everyone reduce psychiatric medications by exactly 10% each month.

That can be a useful conceptual starting point in some circumstances, but hyperbolic tapering is not synonymous with a universal 10% schedule.

Medication, dose, duration of treatment, previous withdrawal experiences, other medications, medical conditions, patient preference, and response to each reduction all matter.

For benzodiazepines, for example, current multidisciplinary guidance generally suggests initial reductions of approximately 5–10% every 2–4 weeks for many physically dependent patients, while emphasizing that some people need substantially slower schedules and that the taper should be adjusted according to response.

The objective isn't to taper as quickly as possible.

The objective is to find a pace your nervous system can tolerate safely.

A Personalized Approach to Medication Reduction

Medication discontinuation does not have to be an all-or-nothing decision.

For some people, the goal is complete discontinuation. For others, it may be finding the lowest effective dose that maintains stability while reducing side effects or medication burden.

A thoughtful taper may involve:

  • Reviewing why the medication was originally prescribed

  • Evaluating how long you have taken it

  • Understanding previous attempts to reduce it

  • Distinguishing withdrawal from recurrence of the underlying condition

  • Making progressively smaller reductions when appropriate

  • Monitoring symptoms between reductions

  • Holding the dose longer when the nervous system needs additional time

  • Using liquid or compounded formulations when clinically appropriate

  • Adjusting the plan rather than following a rigid calendar

Most importantly, tapering should be collaborative rather than something you have to endure according to a predetermined schedule.
